Hospice Team Members




You, our patient, and your family, are the most important members of the Hospice team. You will determine with the Hospice of Reno County, Inc. care team the kind of care that will be given. You have the option to discontinue Hospice care at any time. The Hospice team will rely on you and/or your family to let us know of needs, concerns, and/or questions.

Physician
It is through the patient's physician that referrals are made to Hospice of Reno County, Inc. The patient's primary physician continues to plan an important role in the care. The patient's physician will direct the plan of care, unless the physician or the patient choose otherwise.

Medical Director
The Hospice Medical Director certifies admission to hospice care along with the physician. He/she provides oversight to the plan of care and consultation to the medical team. The patient's primary physician continues to direct the care unless he/she chooses to transfer care to the Medical Director.

Case Manager
The Hospice Registered Nurse (RN) acts as the Case Manager. He/she works closely with the physician to ensure that the patient is kept as physically comfortable as possible. Pain control and symptom management will be of primary concern. The Case Manager makes scheduled visits. Hospice offers consultation by phone and support to the patient and family on a 24-hour, seven day a week basis. For hospice patients and their families, help is just a phone call away.

Home Health Aide
The Home Health Aides assist with the personal care of the patient, such as baths, shampoos, and nail care.

Social Worker
The Social Workers provide individual and family counseling to help meet the challenges created by the illness. They also help the patient and family members obtain the resources necessary to care for the patient.

Hospice Chaplain
Our Hospice chaplains ensure that spiritual support is provided if welcomed. The chaplain can serve as a liaison between the family's clergy and Hospice of Reno County, Inc. The chaplain can also provide spiritual care directly to those patients and families requesting support.

Hospice Volunteer
Following extensive training, a volunteer can offer assistance in various ways. These ways include staying with a patient for a limited time so that family members can have some time away, or providing a practical support such as doing errands or assisting with chores.
