Education and preparation for dismissal will involve you, your family and your physician. The criteria for dismissal are:

• Your treatment objectives are met or are no long able to be met.
• A change in your condition requiring care or services not offered by HomeCare or if more appropriate services can be offered by another service.
• The admission criteria are no longer being met.
• Your insurance carrier criteria are no longer being met and you choose not to pay for further services.
• You do not want further care.
