| Patient's Bill of Rights Complaint Procedure Patient Responsibilities |
As a patient receiving services from Hospice of Reno County or HomeCare of Hutchinson, you have a responsibility to complete the following;
- To answer questions about your medical history or present symptoms as completely and accurately as possible.
- To participate in your plan of care and inform the agency's staff if you have difficulty following your plan of care.
- To comply with agency policies and procedures that affect your care.
- To tell your physician or agency staff if you notice changes in your health.
- To notify agency staff of any changes in your care made by your physician.
- To treat agency personnel with respect and courtesy.
- To remain under the care of a physician in order to assure effective medical treatment.
- To notify agency staff if you need to cancel a scheduled visit for any reason.
- To provide a safe environment for the delivery of care as determined by the agency staff.
